Description

The goal of the Transitions in Care (TiC) CIHR multi-Institute initiative is to improve the health and wellness of Canadians by supporting research that transforms the health system to optimize the outcomes of patients experiencing transitions in care.

About the TiC Team Grants

This funding opportunity is one of several planned as part of the Transition in Care (TiC) Initiative. The focus is to support inter-disciplinary teams that are poised and ready to optimize transition in care outcomes. The grants will provide funds to implement, evaluate, spread, and scale pragmatic solutions to problems associated with transitions in care from a clinical, health care or health system perspective.

Application Streams

The TiC team grants are modeled from the Partnerships for Health System Improvement (PHSI) and eHealth Innovation Partnership Program (eHIPP)approaches and therefore utilize a partnership approach where multidisciplinary research teams (including researchers, clinicians, patients, family members, care-givers, decision-makers, and electronic health innovation partners (as required)) will identify ways to develop, implement, and/or scale and spread (share) solutions into practice to optimize outcomes during transitions in care.

As a result, there are two (2) application streams for this funding opportunity:

  • Stream 1: PHSI solutions for transitions in care gaps;
  • Stream 2: eHIPP solutions for transitions in care gaps.
